Фемороацетабулярный импинджмент: естественная история

Реферат

Эта обзорная статья посвящена естественной истории течения фемороацетабулярного импинджмента (ФАИ) тазобедренного сустава. Рассматривается эволюция пониманий и взглядов хирургов на первичный (идиопатический) артроз и отмечается, что на современном этапе первичный артроз на самом деле практически всегда имеет те или иные причины своего развития в виде анатомических вариаций, среди которых — cam- и pincer-деформации, приводящие к импинджменту. Оказалось, что в большинстве крупных работ была доказана достоверная причинно-следственная связь cam-ФАИ не только с артрозом, но и с риском эндопротезирования. Связь pincer-ФАИ с артрозом оказалась неоднозначной. При этом течение ФАИ-артроза, по всей видимости, оказывается не таким агрессивным, как, например, диспластического артроза, и реже требует эндопротезирования. Поскольку cam-ФАИ является достоверным фактором риска артроза и эндопротезирования, то можно было бы предположить, что своевременная хирургическая коррекция ФАИ позволит остановить или замедлить артроз и избежать эндопротезирования. Однако в настоящее время это предположение не доказано, соответственно, нет оснований утверждать, что операция по поводу ФАИ как-либо влияет на его естественную историю в виде конечного исхода тяжелого артроза и эндопротезирования.
