USING RECTANGULAR FEMORAL COMPONENT IN REVISION SURGERY AFTER THA FAILURE RESULTING FROM ASEPTIC LOOSENING

Abstract

The authors analyzed the treatment outcomes in 34 patients who underwent femoral revision surgery with cementless rectangular femoral components. In all cases the indication for the surgery was THA failure resulting from aseptic loosening. Average follow-up time was 23,8 months. There were no cases of aseptic loosening after the revision surgery. The treatment results were evaluated as excellent or good in (85%) 29 patients.

FEATURES OF SURGICAL TREATMENT OF PATIENTS WITH THE EXTRAARTICULAR HIP FRACTURES

Abstract

The authors considered the results of surgical treatment of 73 patients with different types of pertrochanteric fractures according AO-ASIF classification. There were used identical implants for every group of patients. The authors concluded that intramedullary fixation methods preferably for the type A2 and A3 fractures. Bipolar cemented hip arthroplasty is possible for the type A1 fractures.

SURGICAL TREATMENT OF ANKLE AND FOOT TUMORS

Abstract

The experience of treatment and follow-up 47 patients with tumors of ankle and foot is described. The tumors of this anatomic zone are occurred relatively rarely, they are extremely polymorphic that requires carefully observation of patients with this pathology. Correct maintenance the observation algorithm allows in the most of cases (91,5%) to perform the appropriate organopreserving surgical treatment and achieve good and satisfactory results.

TREATMENT OF POSTTRAUMATIC ISCHEMIC CONTRACTURES OF THE THUMB

Abstract

Results of surgical treatment of 21 patients with posttraumatic ischemic contractures of the first finger of a hand were analyzed. The working classification in which there are three degrees of a contracture depending on size of deficiency of a dermal cord of the first interdigital interval was offered. Advanced tactics of surgical treatment of such pathology was developed. Thus, during the first stage we carried out abduction of the first finger by the elimination of cicatrical, muscle and arthrogenic contracture components. At the second stage of treatment, if it was necessary, active opposition of the first finger was restored.

HYSTOMORPHOLOGIC CHANGES IN INJURED MENISCI IN CHILDREN

Abstract

The purpose of the work was studying histological changes in the injured menisci in children. The histological evaluation of injured menisci received during arthroscopy was performed. The prescription of injury varied from several days till 3 years. It was shown that injured fragment of the meniscus is viable up to 3 months since trauma. It was also obvious that active migration of the meniscus cells occur in the injured fragment and microvessels are seen in 50% of the meniscus tissues.

Abstract

The author articulates the notion of carpal joint, describes its functional anatomy. Particular attention is given to the importance of external and internal ligaments providing the dynamic stability of the radiocarpal joint and wrist. All the main ray methods and techniques of X-ray diagnosis of pathological changes in ligaments of this region were characterized. The main role of MRI-studies in the diagnosis of ligament injuries and carpal instability is proved. The authors has drawn conclusion about the need to continue to explore the potential of MRI in the diagnosis of ligamentous injuries of structures wrist and wrist.

DEFORMATIONS OF THE PROXIMAL FEMUR CONSEQUENTLY BENIGN TUMORS IN CHILDREN AND ADOLESCENTS

Abstract

Benign tumors are affecting the proximal part of the femur in 19% of all cases. Their distinctive feature is the relatively late diagnosis, which requiring the necessity, in addition to removing the tumor and bone grafting, to solve the problem of deformations correction. We have discovered the reasons of deformity appearance of the 31 patients aged 3-16 years. The following types of deformation were defined: ductile (12), after pathological fracture (12) and as a result of growth plate dysfunction (7). The correction of deformation, along with the tumour removing was performed at 15 patients: 3 - Oilier disease, 6 - fibrous dysplasia, 4 - bone cysts, 2 - exostotic chondrodysplasia. In 2 cases the correction of deformity was supposed unreasonable by its severity and accompanying pathology, and in 14 - the deformities were considered as permissible, that not required operative adjustment.

DIAGNOSTICS AND TREATMENT OF DISTAL RADIOULNAR JOINT INJURIES

Abstract

We present the experience of treatment of 169 patients with distal radioulnar joint injuries, operated in specialized hand surgery department of the Clinic of traumatology and orthopedics of Russian State Medical University on the base of Moscow city clinical hospital № 4. We have carried out the following operations: closed osteoclasia of the distal epimetaphys fractures, osteotomy of the radius with the external fixator application in 97 (57,4%) patients, percutaneous 2 Kirschner wires fixation of the ulna head to the radius after reposition of dislocation - 38 (22,5%) patients, lavsanoplasty of DRUJ ligaments - 12 (7,1%), fixation of the ulna head to the radius by pin - 7 (4,1%), open reposition of dislocation of the ulna head and K-wire fixation - 8 (4,8%), osteotomy of synostosis of forearm bones, W. Darrach and Sauve-Kapandji operations - 7 (4,1%). Complications were observed in 13 (7,7%) cases. Postponed results (1 year after operation) were studied in 110 patients. We got good results in 95 (86,4%) cases, satisfactory - in 12 (10,9%) cases, unsatisfactory - in 3 (2,7%) cases.

THE METHOD OF OSTEOSYNTHESIS OF SEGMENTAL HUMERAL FRACTURE BY ROD APPARATUS

Abstract

The results of surgical treatment 12 patients with humeral segmental fractures using external fixation by rod apparatuses with reduction nodes, developed by author are analyzed. Good results were achieved in 11 (91,7%) patients, satisfactory - in 1 (8,3%) in time 1-3 years since the operation. Proposed method of osteosynthesis allows to perform an exact closed reduction of bone fragments and to obtain the stable fixation using the whole period of treatment.

METHOD OF TREATMENT OF DISTAL RADUIS POSTTRAUMATIC DEFORMITY

Abstract

A new method for correcting malunions of the distal metaepiphysis of radial bone using cortical-cancellous autologous transplant from its diaphyseal part. The correcting osteotomy with bone autotransplantation of the proposed method and osteosynthesis of radius using plates with angular stability were performed in 8 patients. Treatment results were evaluated according to the scheme Cooney-Krimmer, «DASH»-questionnaire, as well as the scheme developed by the authors. The union of bone fragments obtained after an average of 2,7 + 0,3 months. In a year after surgery excellent results were obtained in 3 patients, good results - in 5.

Abstract

The problems of timeliness and correctness of diagnostics of bone tumours, as well as therapeutic decision deserve the most careful consideration. The present research concerns the detection of criteria of differential diagnostics of giant-cell tumours, osteocystoma and osteosarcoma (according to the literary data). According to the literature the study of clinical and radiologic diagnostics, allowed to work out differential and diagnostic tables of signs and algorithms of diagnostics of giant-cell tumours, osteocystoma and osteosarcoma. It enabled to detect a therapeutic and diagnostic approach to patients with bone tumours.

STATIC SPINAL DEFORMITY SECONDARY TO PELVIC TILT IN CHILDREN AND ADOLESCENTS: DIAGNOSTICS AND METHODS OF CORRECTION

Abstract

For the purpose of the diagnostics of spinal deformity 2,300 children and adolescents were observed by computer optical topography, tilts of the pelvis were identified in 800 of them. As a result of the evaluation of the initial state of the spine and pelvis, performed by computer optical topography diagnostics 5 types of static deformations of spine and pelvis were defined. The criteria for evaluating distortions of the pelvis, frontal deformation and inclination of spine axis were described. Instrumental study with computer optical topography diagnostics in conjunction with clinical examination reveals various kinds of disturbances of posture, feet condition and functional biomechanical changes in the pelvic region.
