Celecoxibum systemic administration after primary arthroplasty in combination with local infiltration anesthesia has a number of advantages. Purpose of the study. To evaluate the efficiency of postoperative pain relief by Celecoxibum in primary hip arthroplasty using standard and minimally invasive approaches as well as to analyze the pain syndrome dynamics by a method of local infiltration anesthesia (LIA).</p><p><italic>Material and methods.</italic> The study included 91 patients with hip arthritis of grade III-IV without severe anatomical alterations who underwent primary hip arthroplasty in the Vreden Research institute of traumatology and orthopaedics during 2017. Patients were randomized by type of surgical approach used (standard or minimally invasive), the authors compared monotherapy by Celecoxibum with a multimodal anesthesia which included systemic Celecoxibum administration in combination with LIA. Additional evaluation of two LIA methods was conducted in the group of patients with standard approach.</p><p><italic>Results.</italic> Pain syndrome dynamics was comparable in all groups — minimal intensity within first day after the surgery and almost complete absence of pain on day 7. However, on day 3 variance between the subgroups increased: the best outcome was observed in the group of minimally invasive approach combined with LIA VAS score was 1.0 (95% CI, 0–1); in the group with standard approach without LIA VAS score was 4.5 (95% CI, 2–7); in the group with standard approach with a single LIA VAS score was 2.6 (95%CI, 0–5); in the group with standard approach with threestage LIA VAS score was 2.4 (95%CI, 0–5); in the group with minimally invasive approach without LIA VAS score was 3,9 (95% CI, 0–2). On day 7 the parameters in all groups demonstrated no statistically significant variances with a minor elevation in cases when LIA was not performed and with average VAS score of 0.8 (95% CI, 0–4). By day 14 pain syndrome was absent or within a range of 1 point in all patients, by this treatment stage Celecoxibum medication was stopped in 37 out of 91 patients (40.7%).</p><p><italic>Conclusion.</italic> Administration of Celecoxibum per os (200 mg twice per 24 hours) in combination with LIA by Ropivacaine and dexamethasone solution in primary hip arthroplasty provides fast relief of pain facilitating early mobilization of patients and accelerated rehabilitation. Celecoxibum safety in therapeutic dose was also proven.</p></abstract><trans-abstract xml:lang="ru"><p><italic>Введение.</italic> Являясь одним из представителей класса высокоселективных ингибиторов ЦОГ-2, целекоксиб специфически ингибирует функционирование ЦОГ-2 in vivo, не влияя на защитное действие ЦОГ-1. Его системное применение после первичного эндопротезирования в сочетании с локальной инфильтрационной анестезией имеет ряд преимуществ.</p><p><italic>Цель исследования</italic> — оценить эффективность послеоперационного обезболивания целекоксибом при первичном эндопротезировании тазобедренного сустава с использованием стандартного и мини-инвазивного доступов, а также проанализировать динамику изменения болевого синдрома с использованием методики локальной инфильтрационной анестезии (ЛИА).</p><p><italic>Материал и методы.</italic> Выполнено проспективное рандомизированное исследование, в которое включен 91 пациент в возрасте от 29 до 78 лет (средний возраст 59,7 лет, 95% ДИ от 57,6 до 67,7) с коксартрозом III–IV стадии по классификации Kellgren-Lawrence без выраженных анатомических нарушений. Рандомизация пациентов осуществлялась по типу использования хирургического доступа (стандартный или мини-инвазивный). Проведено сравнение монотерапии целекоксибом с мультимодальной схемой анальгезии, включающей системное использование целекоксиба в сочетании с локальной инфильтрационной анестезией. В группе пациентов стандартного доступа выполняли дополнительную оценку двух методик локальной инфильтрационной анестезии. Таким образом, были сформированы 5 групп сравнения: стандартный доступ без ЛИА, стандартный доступ с одномоментной ЛИА, стандартный доступ с трехмоментной ЛИА, МИС без ЛИА, МИС с ЛИА.</p><p><italic>Результаты.</italic> Во всех группах динамика болевого синдрома была сопоставимой: с максимальной выраженностью в первые сутки после операции и с практически полным отсутствием болевых ощущений на 7-й день. Однако на 3-и сутки разница между группами увеличилась. Лучший результат наблюдался в группе мини-инвазивного доступа в сочетании с ЛИА — 1,0 балл (95% ДИ, 0–1), в наблюдениях со стандартным доступом без ЛИА показатели ВАШ составили 4,5 балла (95% ДИ, 2–7), при стандартном доступе в сочетании с одномоментной ЛИА — 2,6 балла (95% ДИ, 0–5), в группе стандартного доступа с трехмоментной ЛИА — 2,4 балла (95% ДИ, 0–5), при мини-инвазивном доступе без ЛИА — 3,9 балла (95% ДИ, 0–2). На 7-й день показатели во всех группах не имели статистически значимых различий с небольшим превышением в тех случаях, когда не выполнялась локальная инфильтрационная анестезия и в среднем составили 0,8 (95% ДИ, от 0 до 4). К 14-му дню болевой синдром отсутствовал или был в пределах 1 балла у всех пациентов, к этому моменту 37 из 91 пациента (40,7%) прекратили прием целекоксиба.</p><p><italic>Вывод.</italic> Применение целекоксиба per os в сочетании с ЛИА раствором ропивакаина и дексаметазона при первичном эндопротезировании тазобедренного сустава обеспечивает быстрое купирование болевого синдрома, способствует ранней активизации пациентов и их ускоренной реабилитации.
