This term means lateral displacement of outer calcaneal wall at the moment of fracture, narrowing of anatomical space under the lateral malleolus and compression of soft tissues in this region, including tendons of short and long peroneal muscles. This leads to chronic traumatization of tendons, alteration of their normal tracking and development of tendinitis and tenosynovitis. At this moment there are no articles in foreign or Russian literature describing how prolonged traumatization influences the internal structure of the tendons. The purpose of this study was to evaluate the morphological changes in structure of peroneus brevis tendon after different duration of compression between outer wall of calcaneus and the tip of the lateral malleolus in patients with calcaneal malunion.</p><p><bold>Materials and methods.</bold> Fifteen patients with calcaneal malunion and lateral impingement syndrome were treated operatively between 2016 and 2017. To confirm the lateral impingement syndrome, the authors performed clinical examination and AP x-rays of ankle joint. Two peroneus brevis tendon specimens were obtained intraoperatively in each of 15 patients: one specimen from compressed and one from non-compressed area. Obtained specimens were histologically examined according to standard protocol.</p><p><bold>Results.</bold> Microscopically all specimens showed separation of collagen bundles with loose connective tissue degeneration, increase of vascularization and inflammation. The degree of these changes differed according to the compression duration. This allowed us to analyze the dynamics of these changes.</p><bold>Conclusion. </bold>The morphological changes in structure of peroneus brevis tendon during the compression between outer wall of calcaneus and the tip of the lateral malleolus correspond with dynamics of common pathologic reactions. Early stages showed signs of mechanical damage of bundles and inflammation. In prolonged impingement the intensity of inflammation decreases, but the connective tissue degeneration of the tendon continues with lipoid infiltration.</abstract><trans-abstract xml:lang="ru"><p/><p><bold>Введение.</bold> Одной из основных причин болевого синдрома у пациентов с последствиями переломов пяточной кости является латеральный импинджмент-синдром, который приводит к хронической травматизации сухожилий, развитию тендинита и теносиновита.</p><p><bold>Цель исследования </bold>— оценить морфологические изменения в структуре сухожилия короткой малоберцовой мышцы после сдавления между наружной стенкой пяточной кости и верхушкой наружной лодыжки у пациентов с неправильно сросшимися переломами пяточной кости.</p><p><bold>Материал и методы. </bold>В период с 2016 по 2017 г. было прооперировано 15 пациентов по поводу неправильно сросшегося перелома пяточной кости, сопровождающегося латеральным импинджмент-синдром. Средний срок от момента травмы составил 31,2 мес. (от 9 до 129 мес.). У каждого из 15 пациентов во время оперативного вмешательства выполнялась биопсия сухожилия короткой малоберцовой мышцы из двух зон — непосредственно из места сдавления и из вышележащих отделов, не подверженных сдавлению. Затем выполнялось гистологическое исследование полученного материала по стандартному протоколу.</p><p><bold>Результаты.</bold> В результате микроскопического исследования в материале было выявлено повреждение коллагеновых волокон, неоваскуляризация, признаки воспаления с клеточной инфильтрацией, дегенерация с замещением волокнистой соединительной тканью. Степень выраженности этих процессов различалась в зависимости от длительности патологического процесса, что позволило проанализировать данные изменения в динамике.</p><bold>Вывод. </bold>Морфологические изменения в структуре сухожилия короткой малоберцовой мышцы в ходе сдавления между латеральной стенкой пяточной кости и верхушкой наружной лодыжки в целом соответствуют динамике общепатологических процессов — на ранних сроках это признаки механического повреждения волокон сухожилия и воспаления, а при длительном течении патологического процесса — стихание воспалительных процессов и превалирование дегенеративных изменений.</trans-abstract><kwd-group xml:lang="en"><kwd>impingement-syndrome</kwd><kwd>calcaneal fractures</kwd><kwd>peroneus brevis tendon</kwd><kwd>tendinitis</kwd><kwd>chronic inflammation</kwd><kwd>neovascularization</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>перелом пяточной кости</kwd><kwd>импинджмент-синдром</kwd><kwd>тендинит</kwd><kwd>патологическая анатомия малоберцовых сухожилий</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>1.
