Restoration of weight bearing ability and creation of conditions for ulcer healing is challenging for trauma and orthopedic surgeons, especially because of the need to restore bone tissue defects.</p> <p><bold><italic>Aim</italic></bold> <bold><italic>of</italic></bold> <bold><italic>the</italic></bold> <bold><italic>review</italic></bold> — to compare modern methods of replacement of large bone tissue defects of the distal segment of the lower limb in patients with Charcot neuropathic osteoarthropathy.</p> <p><bold><italic>Methods</italic></bold><bold><italic>.</italic></bold> The articles were searched in PubMed/MEDLINE, Google Scholar, Scopus, eLIBRARY, CyberLeninka databases. The search was performed using the following keywords and word combinations: Charcot foot, diabetic neuropathic osteoarthropathy and bone grafting. Full-text articles in Russian and English were selected. The search was carried out without chronological limitations due to the small number of published studies and specific subject of research.</p> <p><bold><italic>Results</italic></bold><bold><italic>. </italic></bold>The possibilities of bone defect replacement in Charcot foot are poorly described in the world literature. The narrow specificity and relative infrequency of this pathology is the probable reason for the small number of publications and the lack of studies with a high level of evidence.</p> <p><bold><italic>Conclusion</italic></bold><bold><italic>. </italic></bold>Bone autografting for lesions of the medial or lateral midfoot column and Ilizarov tibial lengthening to compensate for limb shortening after reconstructive surgeries at the level of the hindfoot and ankle joint are the most commonly used methods of bone defect replacement in patients with Charcot foot. Free bone autografting (possibly with demineralized bone matrix) is the method of choice for small defects of the lateral or medial column of the midfoot.</p></abstract><trans-abstract xml:lang="ru"><p><bold><italic>Актуальность.</italic></bold> Во всем мире растет количество больных сахарным диабетом, в том числе с его тяжелыми осложнениями, такими как диабетическая нейроостеоартропатия. Восстановление опороспособности, создание условий для заживления язв являются сложной задачей для травматологов-ортопедов, особенно в связи с необходимостью компенсировать дефект костной ткани.</p> <p><bold><italic>Цель обзора</italic></bold> — сравнить современные методики замещения объемных дефектов костной ткани дистального сегмента нижней конечности у пациентов с диабетической нейроостеоартропатией.</p> <p><bold><italic>Материал и методы.</italic></bold> Поиск статей проводился в базах данных PubMed/MEDLINE, Google Scholar, Scopus, eLIBRARY, CyberLeninka. Поиск осуществлен по следующим ключевым словам и словосочетаниям: стопа Шарко, диабетическая остеоартропатия и костная пластика; Charcot foot, diabetic arthropathy and bone grafting. Отбирались полнотекстовые статьи на русском и английском языках. Поиск проводился без хронологических ограничений в связи с малым количеством опубликованных работ и узкой тематикой.</p> <p><bold><italic>Результаты. </italic></bold>Возможности замещения костных дефектов при стопе Шарко в мировой литературе описаны довольно скудно. Узкая специфика и относительная редкость патологии является вероятной причиной малого количества публикаций, отсутствия исследований с высоким уровнем доказательности.</p> <p><bold><italic>Заключение.</italic></bold> Костная аутопластика при поражении медиальной или латеральной колонны среднего отдела стопы и удлинение голени по Илизарову для компенсации укорочения конечности после реконструктивных операций на уровне заднего отдела стопы и голеностопного сустава являются наиболее часто применяемыми методами замещения костных дефектов у пациентов со стопой Шарко.
