Hip dislocation is the key problem in patients with severe cerebral palsy (GMFCS IV, V) older than 10 years that affects life quality and limits functional capabilities. In the present study the authors evaluated the efficiency of the proximal femoral resection arthroplasty (pfra) and valgus proximal osteotomy of the femur (VPOF) associated with femoral head resection for pain control, improvement of postural management, hygiene and verticalization with total weight-bearing and correction of accompanying orthopaedic deformities. Material and мethods. A retrospective study compared two groups of patients where PFRA (7 cases, 13 hips) or VPOF (14 patients, 23 hips) were performed. Level V of GMFCS was reported in 10 patients, and level IV of GMFCS – in 11 patients. The mean age at time of surgery was 15.3±3.9 y.o. PFRA was performed in 7 cases (13 joints) and VPOF – in 14 patients (23 joints). Results. The authors did not observe any difference between the methods in respect of pain control, postural management, comfortable sitting position and hygiene. The verticalization with total weight-bearing and life quality improvement was achieved only after PVOF with femoral head resection associated with simultaneous knee and foot deformity correction performed according to the principles of Single-Event Multilevel Orthopedic Surgery. Conclusion. Both palliative methods allow to control pain syndrome, to achieve satisfactory postural management, comfortable sitting position and hygiene. But only VPOF with simultaneous knee and foot deformity correction provides possibility to verticalize the patient with weight-bearing using different orthopedic devices.</p></abstract><trans-abstract xml:lang="ru"><p>Введение. У пациентов старше 10 лет с тяжелыми формами ДЦП (GMFCS IV, V) вывих бедра является основной причиной ухудшения качества жизни и значительно ограничивает функциональные возможности. Целью данного исследования является сравнение эффективности артропластической резекции проксимального отдела бедра и проксимальной вальгизирующей опорной остеотомии в сочетании с резекцией головки бедренной кости. Операции оценивались по следующим критериям: коррекция болевого синдрома, улучшение условий для постурального менеджмента, удобства выполнения гигиенических процедур, а также возможности достижения вертикализации и коррекции сопутствующих ортопедических деформаций конечностей, выполненных по принципам одномоментных многоуровневых ортопедических вмешательств. Материал и методы. Сравнение произведено между двумя группами пациентов, которым выполнялись данные вмешательства. В 10 случаях пациенты представляли V уровень GMFCS, в 11 случаях – IV уровень. Средний возраст на момент вмешательства составил 15,3±3,9 лет (от 10 до 27 лет). Артропластическая резекция выполнялась в 7 случаях (13 суставов), вальгизирующая опорная остеотомия – у 14 пациентов (23 сустава). Результаты. Эффективность сравниваемых хирургических вмешательств с точки зрения купирования болевого синдрома, улучшения условий для гигиенических процедур и увеличения комфортности в позиции сидя оказалась одинаковой. Однако вальгизирующая остеотомия в сочетании с резекцией головки и шейки бедра имела преимущества, так как обеспечивала условия для пассивной вертикализации пациентов с опорой на нижние конечности. Одновременное устранение деформаций и контрактур коленного и голеностопного суставов, а также стопы также явилось обязательным условием для вертикализации и повышения качества жизни пациентов. Заключение. Оба рассматриваемых типа паллиативных вмешательства эффективны с точки зрения контроля болевого синдрома и устранения порочного положения бедра, но только вальгизирующая опорная остеотомия бедренной кости в сочетании с резекцией головки бедра и коррекцией контрактур коленного и голеностопного суставов и деформаций стоп обеспечивает условия для пассивной полноценной вертикализации и повышения качества жизни. </p></trans-abstract><kwd-group xml:lang="en"><kwd>cerebral palsy</kwd><kwd>hip dislocation</kwd><kwd>palliative hip surgery</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>детский церебральный паралич</kwd><kwd>вывих бедра</kwd><kwd>паллиативные хирургические вмешательства</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>1.Затравкина Т.Ю., Норкин И.А.
