The treating of clubfoot by Ponseti gives promising results and complete correction of the deformity in 90-95% of patients sparing them from extensive surgery. In the Yaroslavl Regional Children's Hospital from May 2006 to December 2007 54 children (83 feet) with severe congenital clubfoot were treated. Treatment for all patients was initiated at the age of 1 year. With remote monitoring for 5 years recurrences were noted in 19.3% of cases. Despite its high efficacy in patients under the age of three years, non-compliance of brace wearing results in the number of recurrences and require re-gypsum. In older children (3 to 5 years) the treatment by Ponseti is supplemented by transfer of tibialis anterior muscle tendon. In cases of postoperative recurrence of the treatment program is complemented by local releases and arthrodesis.</p></abstract><trans-abstract xml:lang="ru"><p>Врожденная косолапость является одной из самых часто встречающихся патологий опорно-двигательного аппарата. Метод лечения косолапости, предложенный И. Понсети, позволяет получить обнадеживающие результаты и полную коррекцию деформации у 90-95% пациентов, избавляя их от обширных хирургических вмешательств. В Областной детской клинической больнице г. Ярославля с мая 2006 по декабрь 2007 г. прошли лечение 54 ребенка (83 стопы) с врожденной косолапостью тяжелой степени. Лечение всех пациентов было начато в возрасте до 1 года. При отдалённом наблюдении в течение 5 лет рецидивы были отмечены в 19,3% случаев. Несмотря на высокую эффективность лечения у пациентов в возрасте до трех лет, несоблюдение протокола ношения брейсов вызывает увеличение числа рецидивов косолапости и требует повторного гипсования. В старшем возрасте (от 3 до 5 лет) лечение дополняется пересадкой сухожилия передней большеберцовой мышцы.
