When studying the literature, we identify many factors that together influence the outcome of revision ACL reconstruction, but are not always taken into account by surgeons.</p> <p><bold><italic>Aim</italic></bold> <bold><italic>of</italic></bold> <bold><italic>the</italic></bold> <bold><italic>review</italic></bold> — is to perform a systematic review of studies that evaluated the outcomes of revision anterior cruciate ligament reconstructions and to identify optimal solutions to the most common problems encountered in preoperative planning.</p> <p><bold><italic>Methods</italic></bold><bold><italic>.</italic></bold> Information search was performed in the eLIBRARY, PubMed, and Scopus databases for the period from 2013 to 2022. Studies describing the main aspects of revision ACL reconstruction were selected for analysis. Inclusion criteria were the following: mean patient follow-up period of no less than 12 months, number of observations of no less than 10 cases. After evaluation of 898 articles, 22 articles were included in the systematic review.</p> <p><bold><italic>Results</italic></bold><bold><italic>.</italic></bold> Five main factors that should be taken into account in preoperative planning of revision ACL reconstruction have been identified: choice of the graft, necessity of reconstruction of the anterolateral complex, correction of tibial plateau deformity in the sagittal plane, determination of indications for one- or two-stage intervention, method of bone defect replacement in two-stage surgical treatment.</p> <p><bold><italic>Conclusion</italic></bold><bold><italic>. </italic></bold>The patient’s autogenous tissues should be preferred when choosing a graft. Correction of excessive anteroposterior tibial plateau inclination angle is performed only at the second revision intervention if the inclination angle exceeds 12º. Reconstruction of the anterolateral complex should be performed in young, active patients who are involved in pivot sports and in case of severe anterior instability. When determining the possibility of performing revision ACL reconstruction in one or two stages, the canal diameter is not a crucial parameter, as it is necessary to take into account the possibility of fusion of the canals from the previous surgery with the newly created ones. If the canals have correct entry points, one-stage revision reconstruction can be performed with the canal width not exceeding 10 mm and depending on the expected diameter and type of the prepared tendon graft. Bone grafting of the secondary dilated canals in two-stage intervention can be carried out using any material, but allogenous bone or synthetic grafts have certain advantages.</p></abstract><trans-abstract xml:lang="ru"><p><bold><italic>Актуальность. </italic></bold>Количество первичных реконструкций передней крестообразной связки (ПКС) ежегодно увеличивается, что обуславливает рост ревизионных вмешательств из-за несостоятельности трансплантата ПКС. При изучении литературных данных выявляется множество факторов, которые в совокупности влияют на исход ревизионной реконструкции ПКС, но не всегда учитываются хирургами.</p> <p><bold><italic>Цель исследования</italic></bold> — провести систематический обзор исследований, в которых оценивались результаты ревизионных реконструкций передней крестообразной связки и определить оптимальные решения наиболее часто встречающихся задач, возникающих при предоперационном планировании.</p> <p><bold><italic>Материал и методы. </italic></bold>Проведен информационный поиск в базах данных eLIBRARY, PubMed и Scopus за период с 2013 по 2022 г. Для анализа отобраны исследования, в которых описывались основные аспекты ревизионной реконструкции ПКС. Критерии включения: средний срок наблюдения пациентов не менее 12 мес., количество наблюдений не менее 10 случаев. После оценки 898 статей в систематический обзор было включено 22 статьи.</p> <p><bold><italic>Результаты. </italic></bold>Выявлено пять основных факторов, которые нужно учитывать при предоперационном планировании ревизионной реконструкции ПКС: выбор трансплантата, необходимость реконструкции переднелатерального комплекса, коррекция деформации плато большеберцовой кости в сагиттальной плоскости, определение показаний к одно- или двухэтапному вмешательству, метод замещения костных дефектов при двухэтапном оперативном лечении.</p> <p><bold><italic>Заключение. </italic></bold>Предпочтение при выборе трансплантата должно отдаваться аутотканям пациента. Коррекция избыточного переднезаднего угла наклона плато большеберцовой кости выполняется только при повторном ревизионном вмешательстве, если угол наклона превышает 12º. Восстановление переднелатерального комплекса должно выполняться молодым, активным пациентам, которые занимаются «поворотными» видами спорта, а также при наличии выраженной передней нестабильности. При определении возможности выполнения ревизионной пластики ПКС в один или два этапа диаметр канала не является основополагающим параметром, так как необходимо учитывать возможность слияния каналов от предшествующей операции с вновь проведенными. При наличии каналов с корректными точками входа одноэтапная ревизионная пластика может быть выполнена при ширине канала не более 10 мм и в зависимости от предполагаемого диаметра и вида подготовленного сухожильного трансплантата.
