The etiology of pain and myelopathy at degenerative disease is cascade process. Imaging features and clinical correlates in pain and syndromes are presented. The role of imaging is to provide accurate morphologic information, root entrapment at first. MRI is the preferred examination in the assessment of disc herniation, although it has some limitations. Decision making should be based on imaging and clinical manifestations.</p></abstract><trans-abstract xml:lang="ru"><p>Обсуждаются вопросы патогенеза дегенеративных заболеваний позвоночника и стадии развития клинических проявлений. Показано, что дегенеративные заболевания позвоночника представляют собой каскадный процесс, развивающийся с течением времени. Клинические проявления обусловлены сложным комплексом изменений, включающих остеохондроз, спондилёз, остеоартроз, которые часто усугублены врождённой предрасположенностью. Диагностические исследования нацелены на выявление этих изменений. Выявление компрессии корешка особенно важно для выбора тактики лечение.
