Traditional osteosynthesis using buttress plates carries a certain complication rate and often necessitates implant removal.</p> <p>The aim of the study — to compare the anatomical and functional outcomes of the novel osteosynthesis technique versus the conventional technique for post-traumatic impression deformities of the posterior aspect of the lateral tibial condyle.</p> <p>Methods. A prospective cohort study with retrospective control was conducted. The prospective group included 25 patients with post-traumatic deformities of the posterolateral tibial condyle who were operated on using a newly developed technique. The retrospective group comprised 40 patients with similar pathology who underwent conventional surgery via an anterolateral approach with a proximal lateral tibial plate. Outcomes were assessed using the KSS, WOMAC, and VAS scores, as well as radiography and computed tomography (CT) at 3, 6, and 12 months postoperatively.</p> <p>Results. In the prospective group, all functional scores improved significantly: the median KSS increased from 50.0 to 92.0, the WOMAC score decreased from 40.0 to 6.0, and the VAS score dropped to 0.0 (p &lt; 0.001). The median residual impression on CT was 0.0 mm (complete reduction achieved in 72% of patients). Compared with the retrospective group, the prospective group showed significantly better outcomes: KSS — 92.0 vs. 69.5, WOMAC — 6.0 vs. 37.0, VAS — 0.0 vs. 2.0 (p &lt; 0.001), and residual impression on CT — 0.0 vs. 4.0 mm (p &lt; 0.001). The complication rate was 8.0% in the prospective group versus 25.0% in the retrospective group (p = 0.109).</p> <p>Conclusion. The proposed novel technique proved to be a safe and effective alternative to plate osteosynthesis for post-traumatic impression deformities of the posterior aspect of the lateral tibial condyle, providing complete impression reduction, favorable functional outcomes, and a trend toward fewer complications.</p>
<trans-abstract xml:lang="ru"><p>Актуальность. Предложенная новая техника остеосинтеза является безопасной и эффективной альтернативой традиционному способу с применением пластин при лечении посттравматических импрессионных деформаций заднего отдела латерального мыщелка большеберцовой кости, обеспечивая полное устранение импрессии, высокие функциональные результаты и тенденцию к снижению числа осложнений.</p></trans-abstract><trans-abstract xml:lang="zh"><p/></trans-abstract><kwd-group xml:lang="en"><kwd>post-traumatic impression deformity</kwd><kwd>lateral tibial condyle</kwd><kwd>posterolateral tibial plateau</kwd><kwd>osteosynthesis</kwd><kwd>autograft</kwd><kwd>fibula</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>посттравматическая импрессионная деформация</kwd><kwd>латеральный мыщелок большеберцовой кости</kwd><kwd>заднелатеральный отдел плато</kwd><kwd>остеосинтез</kwd><kwd>аутотрансплантат</kwd><kwd>малоберцовая кость</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Беленький И. А.
