The article presents data that are the result of a long-term study (over 1 year) of the effect of a polymer gel based on polyvinyl pyrrolidone with antibiotics on the cells of the bone marrow, soft tissues, and parenchymal organs of rabbits after its implantation with a surgical needle in the medullary canal of the femur. The aim of the work is to assess the effect of an experimental polymer hydrogel with prolonged antimicrobial activity on the histoarchitectonics of parenchymal organs and perifocal tissues one year after its implantation in the distal epiphysis of the femur of laboratory animals (rabbits). Materials and methods. The study used a surgical method of introducing a gel-filled needle into the femur, followed by monitoring the hematological parameters of the rabbit’s blood during the experiment and a histological examination at the end. Results: no deviations of clinical and biochemical blood parameters from reference values were established, which is confirmed by the preserved histoarchitectonics of parenchymal organs during histological examination. As a result of the analysis of histological sections of bone and soft tissue samples of the femur in the area of the gel-impregnated needle implantation, it was revealed that fragments of gel (456.23 – 34108.29 μm<sup>2</sup>) are present throughout the year both in the femur area and in the marrow canal among the red bone marrow cells. Over time, a capsule (~ 10 μm thick) consisting of collagen fibers forms around the particles and between the muscle fibers in the operated area of the thigh. Subsequently, in the cavity of the bone marrow canal, the capsule was observed to be compacted and thickened to 17.5 μm with signs of its heterotopic ossification. According to the test results, in the bone marrow canal, around the gel particles, the presence of lymphocytes was revealed in the form of a thin demarcation zone with a single, perifocal presence of giant multinucleated foreign body cells (1-2 cells per 3 gel particles), containing fragments of the gel in their cytoplasm. No inflammatory reaction was detected to the prolonged presence of the gel in the soft tissues of the thigh in the area of the operation, and in the marrow canal perifocal from the surgical needle place. In dynamics, the development of connective tissue around the gel particles (in the soft tissues of the thigh and in the marrow canal) with its subsequent consolidation in the marrow canal was demonstrated, and it was also established that the gel does not have a toxic effect on bone marrow cells and structures of parenchymal organs.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Актуальность:</bold> Изучение влияния геля на окружающие ткани важно для оценки отсроченных осложнений при использовании такой композиции для предотвращения развития послеоперационных осложнений в месте оперативного вмешательства при репозиции переломов и протезировании элементов костной ткани. В статье представлены данные, являющиеся результатом продолжительного исследования (в течение 1 года) влияния полимерного геля на основе поливинилпирролидона с антибиотиками на клетки костного мозга, мягких тканей и паренхиматозных органов кроликов после его имплантации с хирургической спицей в костномозговой канал бедренной кости. <bold>Цель</bold> работы – оценка влияния экспериментального полимерного гидрогеля с пролонгированной антимикробной активностью на гистоархитектонику паренхиматозных органов и перифокальных тканей через год после его имплантации в область дистального эпифиза бедренной кости лабораторных животных (кроликов). <bold>Материалы и методы.</bold> В работе использован хирургический метод введения спицы с гелем в бедренную кость, с последующим контролем гематологических показателей кроликов в течении эксперимента и гистологическим исследованием в завершении. <bold>Результаты:</bold> установлено отсутствие отклонений клинических и биохимических показателей крови от референсных значений, что подтверждается сохранённой гистоархитектоникой паренхиматозных органов при гистологическом исследовании. В результате анализа гистологических срезов образцов кости и мягких тканей бедра в области имплантации спицы с гелем было выявлено, что фрагменты геля (456,23 – 34108,29 мкм<sup>2</sup>) присутствуют на протяжении года и в области бедра, и в костномозговом канале среди клеток красного костного мозга. С течением времени вокруг его частиц, между мышечными волокнами в оперируемой области бедра формируется капсула (толщиной ~ 10 мкм), состоящая из коллагеновых волокон. Впоследствии, в полости костномозгового канала, замечено уплотнение и утолщение данной капсулы до 17,5 мкм с признаками её гетеротопической оссификации. По результатам испытаний в костномозговом канале вокруг частиц геля было выявлено присутствие лимфоцитов в виде тонкой демаркационной зоны с единичным, перифокальным присутствием гигантских многоядерных клеток инородного тела (1 – 2 клетки на 3 частицы геля), содержащих фрагменты геля в своей цитоплазме. Воспалительной реакции на длительное присутствие геля в мягких тканях бедра в области операции, и в костномозговом канале перифокально от ложемента хирургической спицы не выявлено.
