Traditionally, cemented femoral components have been used; however, they are associated with a high rate of long-term complications.</p> <p>The aim of the study — to evaluate the clinical and radiographic outcomes of primary total hip arthroplasty in patients with Dorr type C femoral morphology.</p> <p>Methods. This single-center retrospective stratified study included 76 patients with Dorr type C femoral bone who underwent primary total hip arthroplasty between 2018 and 2024. Patients were divided into three groups according to the type of femoral component fixation: Group I (n = 23) — cemented; Group II (n = 34) — cementless; Group III (n = 19) — revision femoral components. The following parameters were assessed: the Barnett-Nordin index, canal-to-calcar ratio (CCR), intraoperative blood loss, operative time, functional outcomes according to the Oxford Hip Score (OHS-48) and the UCLA Activity Score, and the number and type of complications.</p> <p>Results. The mean follow-up period was 44.3±9.2 months, and the mean intraoperative blood loss was 488.8±55.3 ml. Functional outcomes after THA did not differ significantly between the cemented and cementless stem groups. A statistically significant improvement (p = 0.04) was observed among patients who received revision femoral components. ROC analysis was performed for two radiographic parameters — the Barnett-Nordin index and CCR. Threshold values associated with an increased risk of complications were identified: for the Barnett-Nordin index, 0.17 (OR 10.6; 95% CI 2.77-40.84); and for the CCR, 0.92 (OR 9.6; 95% CI 2.50-35.90). Periprosthetic femoral fractures occurred in 6.5% of cases as early complications.</p> <p>Conclusion. There was no statistically significant difference in long-term functional outcomes (beyond 3 years) after total hip arthroplasty between cemented and cementless femoral components in patients with Dorr type C femoral bone. In patients with the Barnett-Nordin index &lt; 0.170 and a CCR &gt; 0.920, the use of cementless revision stems was associated with fewer complications and better functional outcomes in the early postoperative period.</p>
<trans-abstract xml:lang="ru"><p>Актуальность. При значениях морфокортикального индекса &lt; 0,170 и индекса CCR &gt; 0,920 использование бесцементных ревизионных ножек демонстрирует меньшее количество осложнений и лучшие функциональные результаты в раннем послеоперационном периоде.</p></trans-abstract><trans-abstract xml:lang="zh"><p/></trans-abstract><kwd-group xml:lang="en"><kwd>total hip arthroplasty</kwd><kwd>femoral canal</kwd><kwd>cementless revision femoral components</kwd><kwd>Barnett-Nordin index</kwd><kwd>Dorr index</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>эндопротезирование тазобедренного сустава</kwd><kwd>костномозговой канал бедра</kwd><kwd>бесцементные ревизионные бедренные компоненты</kwd><kwd>индекс Barnett-Nordin</kwd><kwd>индекс Dorr</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Шубняков И. В. и др.
