A comprehensive search was conducted in international and Russian databases for publications from 2000 to 2025. Systematic reviews and meta-analyses on proximal interphalangeal joint (PIPJ) surgery were included. The methodological quality was assessed using AMSTAR 2 and ROBIS tools. The degree of the overlap of primary studies across reviews was assessed using the Corrected Covered Area (CCA). Due to substantial heterogeneity in the data, conducting a new quantitative meta-analysis was considered inappropriate.</p> <p><bold>Results.<italic> </italic></bold>Seventeen systematic reviews comprising 211 unique primary studies were included. The overall CCA was 4%; however, substantial overlap was observed within specific intervention categories (joint arthroplasty — 17.3%; hemi-hamate autograft arthroplasty — 31%; arthrodesis — 19%; denervation — 56%). According to AMSTAR 2, six reviews were rated as high quality, nine as moderate, and two as low. Based on ROBIS, the risk of bias was low in ten reviews, unclear in six, and high in one. Following PIPJ arthrodesis, repeated interventions are quite rare. Silicone implant arthroplasty offers the most predictable balance between restoration of motion (approximately 50-60°), pain reduction, and acceptable implant survival (85-94% at 3-5 years), although revision rates remain high. Pyrocarbon implants are associated with higher complication and revision rates compared with silicone implants, without convincing functional advantages. Hemi-hamate autograft arthroplasty achieves a high postoperative range of motion (74°), but is applicable only in carefully selected patients with narrowly defined indications. Denervation leads to significant pain reduction but does not address structural joint pathology and has no effect on joint function. Mobilizing procedures are effective in improving joint motion, but full restoration of flexion is rarely achieved.</p> <p><bold>Conclusions.</bold> Surgical treatment methods for the sequelae of the proximal interphalangeal joint injuries of the fingers yield heterogeneous clinical outcomes and are associated with different complication risks. Therefore, the choice of the optimal procedure depends on the specific clinical scenario. The low overall level of evidence, the absence of direct comparisons, and the heterogeneity of reported outcomes underscore the need for prospective comparative studies and standardized core outcome sets.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Цель обзора</bold> — систематизация и критическая оценка данных систематических обзоров по хирургическому лечению последствий внутрисуставных повреждений проксимального межфалангового сустава пальцев кисти у взрослых.</p> <p><bold>Материал и методы.<italic> </italic></bold>Выполнен зонтичный обзор систематических обзоров (umbrella review) в соответствии с рекомендациями PRIOR 2022 и PRISMA 2020. Поисковая стратегия охватывала международные и российские базы данных за 2000–2025 гг. Включались систематические обзоры и метаанализы исследований, посвященных хирургическим вмешательствам на проксимальном межфаланговом суставе (ПМФС) пальцев кисти. Методологическое качество оценивалось с использованием AMSTAR 2 и ROBIS; степень перекрытия первичных исследований — по Corrected Covered Area (CCA). Количественный метаанализ не проводился из-за выраженной гетерогенности данных.</p> <p><bold>Результаты.</bold> Для анализа отобрано 17 систематических обзоров (211 уникальных первичных исследований). Общий CCA составил 4%, однако внутри отдельных направлений выявлено высокое перекрытие (эндопротезирование — 17,3%; артропластика костным аутотрансплантатом крючковидной кости (hemi-hamate) — 31%; артродез — 19%; денервация — 56%). Качество шести обзоров по AMSTAR 2 расценено как высокое, девяти — умеренное и двух — низкое. По ROBIS в 10 обзорах риск смещения был низким, в 6 — неясным и в 1 — высоким. При артродезе ПМФС частота повторных вмешательств минимальна. Эндопротезирование ПМФС с использованием силиконовых эндопротезов позволяет получить наиболее предсказуемое сочетание сохранения движений в суставе (50–60°), уменьшения боли и приемлемой выживаемости имплантата (85–94% в течение 3–5 лет), однако вероятность повторных вмешательств велика. Использование пироуглеродных имплантатов в сравнении с силиконовыми протезами ассоциировано с более высокой частотой осложнений и ревизий при отсутствии убедительных преимуществ в функциональных исходах. Артропластика hemi-hamate обеспечивает высокий диапазон движений в оперированном суставе (74°), однако метод применяется по строгим показаниям у ограниченного контингента пациентов. Денервация приводит к значительному уменьшению боли, но ее возможности ограничены отсутствием влияния на структуру и функцию сустава. Мобилизирующие операции увеличивают диапазон движений, но не гарантируют полного восстановления.</p> <p><bold>Заключение.</bold> Хирургические методы лечения последствий повреждений проксимального межфалангового сустава пальцев кисти дают неодинаковые клинические результаты и связаны с различными рисками осложнений, поэтому выбор оптимального метода зависит от клинической ситуации.
