Numerous surgical approaches have been proposed; however, none of them completely address the problem of tissue defect elimination when correcting persistent equinus foot deformity.</p> <p><bold>The</bold><bold> </bold><bold>aim</bold><bold> </bold><bold>of</bold><bold> </bold><bold>the</bold><bold> </bold><bold>study</bold><bold> </bold>— to demonstrate a new surgical approach for Achilles tendon reconstruction.</p> <p><bold>Surgical</bold><bold> </bold><bold>technique</bold><bold> </bold><bold>description</bold><bold>.</bold> The surgical approach for Achilles tendon lengthening involved creating an arched incision and sequentially isolating the skin and fascial flaps. Achilles tendon shortening was corrected using Z-plasty. After that, the tendon was covered first with a fascial flap, and then the skin was partially sutured without tension. The remaining soft tissue defect was covered with a skin graft. Patients underwent 6 weeks of plaster immobilization, after which full weight-bearing on the operated leg and physical therapy was allowed.</p> <p><bold>Conclusion</bold><bold>.</bold> The described surgical approach is simple to perform and allows for the simultaneous replacement of tissue defects following the correction of equinus foot deformity.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Актуальность</bold>. Малая подвижность кожи задней поверхности нижней трети голени, а также небольшое количество подкожно-жировой клетчатки и близкое расположение ахиллова сухожилия определяют склонность к формированию дефекта тканей после оперативного вмешательства в этой области. Предложено большое количество оперативных доступов, однако ни один из них не решает проблему устранения дефекта при коррекции стойкой эквинусной установки стопы.</p> <p><bold>Цель исследования </bold>— продемонстрировать новый способ оперативного доступа при реконструкции ахиллова сухожилия.</p> <p><bold>Техника операции.</bold> Оперативный доступ для удлинения ахиллова сухожилия заключался в формировании дугообразного разреза и последовательном выделении кожного, а затем фасциального лоскута. Устраняли укорочение ахиллова сухожилия путем выполнения Z-образной пластики, после чего сначала укрывали сухожильную ткань фасциальным лоскутом, затем частично ушивали кожу, не допуская натяжения тканей. Оставшийся дефект мягких тканей восполняли кожным трансплантатом.
