It is essential to study the factors influencing the complications rate in order to optimize surgical techniques and improve the outcomes.</p> <p><bold><italic>The aim of the study</italic></bold> — to identify the incidence of postoperative complications and risk factors for their development in patients undergoing periacetabular osteotomy.</p> <p><bold><italic>Methods</italic></bold>. The study included 82 patients (89 joints) with hip dysplasia (Crowe I-II grades, Hartofilakidis A and B types), operated between 2007 and 2023. The mean age of the participants was 30.90±8.71 years (95% CI: 29.02-32.79). We analyzed 178 X-rays and 58 CT scans performed one day before surgery and on day 1 after PAO. Radiographic parameters assessed included Wiberg angle, Tönnis angle, Sharp angle, femoral head extrusion index, index of sphericity of femoral head, and retroversion index. The CT scans were used to evaluate the values of AASA (anterior acetabular sector angle), PASA (posterior acetabular sector angle), HASA (horizontal acetabular sector angle), and AcetAV (acetabular anteversion angle).</p> <p><bold><italic>Results. </italic></bold>Identified complications included acetabular overcorrection leading to pincer-type femoroacetabular impingement (24.72%), acetabular undercorrection (13.48%), stress fractures (16.85%), neurological impairments (8.99%), and infectious complications (3.37%). Patients with signs of overcorrection were 2.67 times more likely to require hip replacement (95% CI: 1.41-5.08). Undercorrection was associated with a 4.4-fold increase in arthroplasty risk (95% CI: 1.42-13.70; p = 0.013). An inverse relationship was found between the femoral head sphericity index and the likelihood of hip replacement following PAO: a 1% increase in this index reduced the odds of arthroplasty by 1.28 times (95% CI: 1.09-1.49).</p> <p><bold><italic>Conclusions. </italic></bold>The key factors influencing hip joint survival after periacetabular osteotomy are the accuracy of acetabular correction and the degree of femoral head sphericity. To reduce the rate of complications, it is necessary to develop preoperative criteria for assessing joint congruity, as well as tools for precisely positioning of the acetabular fragment.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Актуальность. </bold>Периацетабулярная остеотомия (ПАО) — эффективный метод предотвращения или отсрочки эндопротезирования тазобедренного сустава у взрослых пациентов молодого возраста. Однако ее выполнение сопряжено с риском развития осложнений. Для оптимизации хирургической техники и улучшения исходов хирургического лечения необходимо изучение факторов, влияющих на частоту развития различных осложнений.</p> <p><bold>Цель исследования </bold>— выявить частоту осложнений после периацетабулярной остеотомии и факторы риска их развития.</p> <p><bold>Материал и методы.</bold> В исследование были включены 82 пациента (89 суставов с дисплазией тазобедренного сустава (степень I–II по Crowe, типы A и B по Hartofilakidis), оперированных в 2007–2023 гг. Средний возраст пациентов составил 30,9±8,7 года (95% ДИ: 29,0 — 32,8). Были проанализированы 178 рентгенограмм и 58 компьютерных томограмм, выполненных за сутки до операции и в 1-е сут. после проведения ПАО. Оценивали следующие рентгенологические показатели: углы Wiberg и Tönnis, угол вертикального наклона вертлужной впадины (ВВ), индексы экструзии головки бедренной кости, сферичности головки бедренной кости и ретроверсии. На томограммах оценивали значения углов AASA (anterior acetabular sector angle), PASA (posterior acetabular sector angle), HASA (horizontal acetabular sector angle) и антеверсии ВВ (AcetAV).</p> <p><bold>Результаты.</bold> Выявлены следующие осложнения: гиперкоррекция положения ВВ с формированием фемороацетабулярного импиджмента типа pincer (24,72%), недостаточная коррекция ВВ (13,48%), стресс-переломы (16,85%), неврологические нарушения (8,99%), инфекционные осложнения (3,37%). Относительный риск необходимости выполнения эндопротезирования увеличивался в 2,67 раза у пациентов с признаками гиперкоррекции (95% ДИ: 1,41–5,08). Недокоррекция ассоциировалась с увеличением риска эндопротезирования в 4,4 раза (95% ДИ: 1,42–13,70; p = 0,013). Выявлена обратная связь между значением индекса сферичности головки бедренной кости и вероятностью выполнения эндопротезирования тазобедренного сустава после проведения ПАО. Увеличение данного индекса на 1% уменьшает шансы выполнения эндопротезирования в 1,28 раза (95% ДИ: 1,09–1,49).</p> <p><bold>Заключение. </bold>Основными факторами, влияющими на выживаемость тазобедренного сустава после периацетабулярной остеотомии, являются точность коррекции вертлужной впадины и степень сферичности головки бедренной кости.
