Conservative treatment methods are effective in the early stages of the disease, but in advanced stages, surgical intervention becomes the primary treatment approach. In recent years, resection interposition and resection suspension-interposition arthroplasty techniques for the first carpometacarpal joint have gained popularity. However, there is a lack of comparative studies on the effectiveness of these techniques based on key indicators.</p> <p><bold>The aim of this study</bold> was to compare the results of surgical treatment for stages II–IV rhizarthrosis using resection suspension-interposition arthroplasty and resection interposition arthroplasty of the first carpometacarpal joint.</p> <p><bold>Methods<italic>. </italic></bold>This prospective cohort study included 85 patients with stages II–IV rhizarthrosis who underwent surgery between 2017 and 2022. Patients were divided into two groups. In the first group (52 patients), resection suspension-interposition arthroplasty of the first carpometacarpal joint was performed using an original technique. In the second group (33 patients), resection interposition arthroplasty was performed. The effectiveness of surgical treatment was assessed based on the intensity of pain using the Visual Analog Scale for Pain (VAS), upper limb function recovery assessed with the Disabilities of the Arm, Shoulder, and Hand (DASH) scale, measurement of the distance between the base of the first metacarpal bone and the distal pole of the trapezium bone on anteroposterior X-rays, and an increase in grip strength of the affected hand at 12 months postoperatively.</p> <p><bold>Results<italic>. </italic></bold>At the 12-month follow-up, the first group showed superior results compared to the second group: a 73% improvement in DASH score (p&lt;0.01); a 76% improvement in VAS (p = 0.000); a 60% increase in cylindrical grip strength (p = 0.010); and a 30% improvement in the measurement of the distance between the base of the first metacarpal bone and the distal pole of the trapezium bone (p&lt;0.01).</p> <p><bold>Conclusion<italic>. </italic></bold>Resection suspension-interposition arthroplasty of the first carpometacarpal joint is more effective than resection interposition arthroplasty in terms of pain relief, improvement in function and grip strength of the affected hand, and preservation of the distance between the base of the first metacarpal bone and the distal pole of the trapezium bone.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Актуальность<italic>.</italic></bold> Ризартроз наиболее часто встречается у людей, чей характер труда и активности ассоциирован с нагрузкой на кисть. На начальных стадиях заболевания эффективны консервативные методы лечения, однако на более поздних стадиях основным методом лечения является оперативное вмешательство. В течение последних лет большую популярность завоевали методики резекционной интерпозиционной и резекционной суспензионно-интерпозиционной артропластики первого запястно-пястного сустава, а также их комбинации. На данный момент отсутствуют сообщения о сравнительных исследованиях эффективности этих методик по основным показателям.</p> <p><bold>Цель исследования</bold> — сравнить результаты хирургического лечения ризартроза II–IV стадий методами резекционной суспензионно-интерпозиционной артропластики и резекционной интерпозиционной артропластики первого запястно-пястного сустава.</p> <p><bold>Материал и методы<italic>. </italic></bold>В проспективное когортное исследование было включено 85 пациентов с ризартрозом II–IV стадий, которым было выполнено 85 хирургических вмешательств за период с 2017 по 2022 г. Пациенты были разделены на две группы. В первой группе (52 пациента) выполняли резекционную суспензионно-интерпозиционную артропластику первого запястно-пястного сустава по оригинальной методике, во второй группе (33 пациента) — резекционную интерпозиционную артропластику. Эффективность хирургического лечения оценивали по результатам оценки интенсивности болевого синдрома по ВАШ, восстановления функции верхней конечности по шкале DASH, измерения расстояния между основанием первой пястной кости и дистальным полюсом ладьевидной кости на рентгенограммах в прямой проекции и увеличения силы цилиндрического хвата пораженной кисти через 12 мес. после оперативного вмешательства.</p> <p><bold>Результаты<italic>. </italic></bold>Через 12 мес. после лечения в первой группе по сравнению со второй группой показатели были лучше: на 73% по шкале DASH (<italic>p</italic>&lt;0,01); на 76% по ВАШ (<italic>p </italic>= 0,000), на 60% при оценке силы цилиндрического хвата (<italic>p</italic> = 0,010); на 30% при оценке расстояния между основанием первой пястной кости и дистальным полюсом ладьевидной кости (<italic>p</italic>&lt;0,01).</p> <p><bold>Заключение<italic>.</italic></bold> Резекционная суспензионно-интерпозиционная артропластика первого запястно-пястного сустава является более эффективной, чем резекционная интерпозиционная артропластика, по следующим показателям: купированию болевого синдрома, улучшению функции и силы цилиндрического хвата пораженной кисти, сохранению расстояния между основанием первой пястной кости и дистальным полюсом ладьевидной кости.</p></trans-abstract><trans-abstract xml:lang="zh"><p/></trans-abstract><kwd-group xml:lang="en"><kwd>rhizarthrosis</kwd><kwd>first carpometacarpal osteoarthritis</kwd><kwd>interposition arthroplasty</kwd><kwd>suspension arthroplasty</kwd><kwd>hand surgery</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>ризартроз</kwd><kwd>остеоартроз первого запястно-пястного сустава</kwd><kwd>интерпозиционная артропластика</kwd><kwd>суспензионная артропластика</kwd><kwd>хирургия кисти</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Peat G., Thomas M.J.
