The rarity of the disease and, in this regard, the lack of doctors awareness about the pathology, late diagnosis and severe complications of the musculoskeletal system emphasize the relevance of clinical case demonstrating. The uniqueness of  the  case  lies  in  the  fact  that  hypophosphatemia,  noted  3  years  after  the  disease  debut,  was  not  taken  into  account.</p><p>Case description. A 45-year-old patient with complaints of muscle weakness, gait disorders, torso deformity and multiple vertebral body fractures that appeared against the background of any somatic diseases absence, a differential diagnosis of metastatic vertebral bodies lesions and secondary osteoporosis complicated by vertebral body fractures was carried out for four years in various hospitals, and was even treated with bisphosphonates. Against this background, the chest deformity increased,  kyphosis  and  remodeling  fractures  of  other  bones  appeared.  The  assessment  of  calcium  and  phosphorus homeostasis  was  first  performed  at  the  4th year  of  the  disease,  but  the  detected  hypophosphatemia  was  not  regarded as  a  manifestation  of  hypophosphatemic  osteomalacia.</p><p>Conclusion. Among  adult  patients  with  multiple  low-energy fractures,  severe  muscle  weakness  and  bone  pain  that  appeared  against  the  background  of  complete  health,  to  exclude hypophosphatemic osteomalacia induced by mesenchymal tumor, it is necessary to include the level of phosphorus in blood and daily urine assessment in the diagnostic algorithm.</p></abstract><trans-abstract xml:lang="ru"><p>Актуальность. Редкость заболевания и обусловленная этим неосведомленность врачей о патологии, поздняя диагностика и тяжелые осложнения со стороны костно-мышечной системы подчеркивают актуальность демонстрации клинического случая. Уникальность случая заключается в том, что гипофосфатемия, отмеченная спустя 3 года от начала  заболевания,  не  была  принята  во  внимание.</p><p>Описание  случая. У  пациента  45 лет  с  жалобами  на  мышечную слабость, нарушение походки, деформацию туловища и множественные переломы тел позвонков, появившиеся на фоне отсутствия каких-либо соматических заболеваний, на протяжении четырех лет в различных медицинских учреждениях  проводился дифференциальный диагноз  метастатического  поражения тел  позвонков  и  вторичного остеопороза, осложненного переломами тел позвонков, и даже было проведено лечение бисфосфонатами по поводу  остеопороза.  На  этом  фоне  увеличивалась  деформация  грудной  клетки,  кифоз,  появились  перестроечные переломы других костей скелета. Оценка гомеостаза кальция и фосфора впервые проведена на 4-м году заболевания, но выявленная гипофосфатемия не была расценена как проявление гипофосфатемической остеомаляции.</p><p>Заключение.Среди взрослых пациентов с множественными низкоэнергетическими переломами, выраженной мышечной слабос тью и болями в костях, появившихся на фоне полного здоровья, для исключения гипофосфатемической  остеомаляции,  индуцированной  мезенхимальной  опухолью,  необходимо  включить  в  алгоритм диагностики оценку уровня фосфора в крови и суточной моче.</p><p> </p></trans-abstract><kwd-group xml:lang="en"><kwd>phosphaturic oncogenic osteomalacia, FGF-23 secreting tumor, phosphaturic mesenchymal tumor, pathological</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>фосфатурическая онкогенная остеомаляция</kwd><kwd>ФРФ-23 секретирующая опухоль</kwd><kwd>фосфатурическая мезенхимальная опухоль</kwd><kwd>костная патологи</kwd><kwd>переломы без травмы</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>1.
