Despite the fact that PJI is more often caused by gram-positive microorganisms, the treatment of patients induced by gram-negative pathogens and fungi is the most complex and associated with the higher recurrence rate. This paper presents a positive two-stage treatment of a patient with fungal periprosthetic infection with a review of current medical literature. Revision, sanation of infection site and implantation of articulating antibacterial spacer was performed in the first stage of treatment. The second stage, which was the implantation of a revision prosthesis, followed in 6 months after removal of infection nidus. Subsequently the authors obtained good functional outcomes and stopping the infection process.</p></abstract><trans-abstract xml:lang="ru"><p>Частота перипротезной инфекции (ППИ) после первичного тотального эндопротезирования коленного сустава составляет от 0,5 до 6%, в то время как после ревизионного возрастает до 13,6%. Несмотря на то, что чаще всего ППИ обусловлена грамположительными микроорганизмами, лечение пациентов с инфекцией, в этиологии которой принимают участие грамотрицательные патогены и грибы, наиболее сложно и ассоциировано с большей частотой рецидивов. В данной статье представлен положительный опыт двухэтапного лечения пациента с перипротезной инфекцией грибковой этиологии с обзором актуальных медицинских публикаций. Первым этапом произведено удаление компонентов эндопротеза, синовэктомия, санация полости сустава с установкой артикулирующего антибактериального спейсера. Через 6 мес. после устранения очага инфекции проведен второй этап хирургического лечения — имплантация ревизионной ортопедической конструкции. При дальнейшем наблюдении пациента достигнуты хорошие функциональные результаты лечения и купирование инфекционного процесса.</p><p>Пациент дал добровольное информированное согласие на публикацию клинического наблюдения.
