However, complications developing after application of rigid fixation systems resulted in active introduction of dynamic implants into clinical practice.</p><p><bold>Purpose of the study</bold> — to compare effectiveness of nitinol and titanium nails for lumbosacral fixation in surgical treatment of degenerative spine diseases.</p><p><bold>Materials and methods.</bold> 220 patients who underwent surgeries in 4 hospitals were randomized into two groups, each consisting of 110 patients (1:1 ratio): a group of patients who underwent stabilization of the vertebral motor segments with rods of nitinol with the required volume of decompression at the operation level and a group of patients who underwent stabilization of the vertebral motor segments with standard rods of titanium with the required volume of decompression at the intervention level. Patients suffered clinically significant spinal canal stenosis in one or two adjacent segments: from L3 to S1. Outcomes were evaluated during three years postoperatively by VAS scale for spine and lower limbs, and by ODI and SF-36 scales.</p><p><bold>Results.</bold> All scales demonstrated better values in both groups of patients, namely, significant decrease of pain syndrome and improvement in mental and physical health. X-ray examination of all patients during the study period demonstrated restoration of lumbar lordosis. Group of patients with dynamic nails featured less complications rate related to metal implants including adjacent segment disease.</p><p><bold>Conclusion.</bold> Transpedicular fixation of lumbosacral spine by nitinol nails is an effective technique allowing to preserve motion along with stable fixation.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Актуальность.</bold> Хирургическая декомпрессия и декомпрессия со стабилизацией чрезвычайно эффективны при лечении стеноза позвоночного канала на уровне поясничного отдела. Однако развивающиеся осложнения на фоне применения жестких стабилизирующий систем привели к активному внедрению динамических имплантатов.</p><p><bold>Цель исследования</bold> — сравнить эффективность применения стержней из нитинола и титановых стержней при хирургическом лечении дегенеративных заболеваний позвоночника с фиксацией пояснично-крестцового отдела.</p><p><bold>Материал и методы.</bold> 220 пациентов, которые получили хирургическое лечение в 4 лечебных учреждениях, были рандомизированы в две группы по 110 человек (соотношение 1:1). Первую группу составили пациенты, которым проводилась стабилизация позвоночно-двигательных сегментов стержнями из нитинола с необходимым объемом декомпрессии на уровне операции. Во вторую группу вошли пациенты, которым выполнялась стабилизация позвоночно-двигательных сегментов стандартными стержнями из титана с необходимым объемом декомпрессии на уровне вмешательства. Пациенты имели клинически значимые стенозы позвоночного канала в одном или двух смежных сегментах: от l3 до S1. Результаты оценивали по ВАШ для спины и нижних конечностей, по шкалам ODI и SF-36 в течение трех лет после операции.</p><p><bold>Результаты.</bold> По всем шкалам было отмечено улучшение в обеих группах пациентов в виде значительного снижения болевого синдрома, улучшения психологического и физического здоровья. При рентгенологическом обследовании у всех пациентов на протяжении исследования отмечено восстановление лордоза поясничного отдела позвоночника.
