The age range is 60-89 years (mean 66 years). The criteria for identifying the leading syndromes: leg pain/back pain with a threshold value of 5 points according to VAS, X-ray criteria for clinical instability by A.A. White and M.M. Panjabi (value &gt;5 points), Cobb angle &gt;10°, markers of sagittal imbalance: Index Barrey (II and III), PT increase above target values, L4-S1 and LL deficiency.</p> <p><bold><italic>Results</italic></bold><bold><italic>.</italic></bold> Symptoms of compression were identified in 97% of patients. Radiculopathy syndrome was detected in 665 (66%) patients with mean leg pain 7 points, neurogenic intermittent claudication — in 319 (31%) patients. Degenerative spondylolisthesis according to radiological criteria was detected in 428 (42%) patients. Degenerative scoliotic deformity had 91 (9%) patients. In accordance with the proposed criteria, the dominant compression syndrome was determined in 624 patients (62%), clinical instability syndrome — in 338 (33%), deformity syndrome with sagittal imbalance — in 51 (5%).</p> <p><bold><italic>Conclusion</italic></bold><bold><italic>.</italic></bold> Syndromic assessment of clinical, neurological and radiological manifestations of degenerative disorders provides the possibility of identifying the dominant syndrome requiring operative surgical treatment and a differentiated approach to choosing the optimal surgical option.</p></abstract><trans-abstract xml:lang="ru"><p><bold><italic>Актуальность.</italic></bold> Выбор метода оперативного лечения при дегенеративной патологии поясничного отдела позвоночника (ПОП) затруднен в связи с отсутствием четких клинико-рентгенологических критериев диагностики и прямой зависимости между тяжестью рентгенологических проявлений заболевания и клинической симптоматикой.</p> <p><bold><italic>Цель исследования </italic></bold>— на основе анализа клинических, неврологических и рентгенологических характеристик пациентов пожилого и старческого возраста с дегенеративной патологией поясничного отдела позвоночника выделить доминирующие клинические и рентгенологические синдромы, определяющие снижение качества жизни и требующие хирургической коррекции.</p> <p><bold><italic>Материал и методы. </italic></bold>С использованием малоинвизивных технологий (изолированные декомпрессии, TLIF, LLIF, ALIF) в период с 2013 по 2017 г. оперировано 1013 пациентов (367 мужчин и 646 женщин) в возрасте 60–89 лет (в среднем 66 лет). Критерии выделения ведущих синдромов: боль в ноге/спине с пороговым значением 5 баллов по ВАШ, рентгенологические критерии клинической нестабильности по A.A. White и M.M. Panjabi (&gt;5 баллов), угол Кобба &gt;10°, маркеры сагиттального дисбаланса: индекс Barrey (II и III), повышение PT выше целевых значений, дефицит L4-S1 и LL.</p> <p><bold><italic>Результаты. </italic></bold>Симптомы компрессии выявлены у 97% пациентов: синдром радикулопатии — у 665 (66%) больных с болями в ноге (в среднем 7 баллов по ВАШ), нейрогенная перемежающаяся хромота — у 319 (31%). Дегенеративный спондилолистез по рентгенологическим критериям выявлен у 428 (42%) пациентов, дегенеративная сколиотическая деформация — у 91 (9%).
